Elston-Ellis modification of the SBR grading system (Nottingham grading system). Fibromatosis: bland appearance with spindle cells arranged in sheets,
There are different "scoring systems" available for determining the grade of a breast cancer. One of these systems is the Nottingham Histologic Score system (also termed “the Elston-Ellis modification of Scarff-Bloom-Richardson grading system”). In this scoring system, there are three factors that the pathologists take into consideration:
BR may be expressed in scores (range 3-9) The score is based on three morphologic features of “invasive no -special-type” breast cancers (degree of Breast and prostate cancers are the most common types of cancer that have their own grading systems. Breast cancer.Doctors most often use the Nottingham grading system (also called the Elston-Ellis modification of the Scarff-Bloom-Richardson grading system) for breast cancer (). Results The 0.7 cm invasive carcinoma was graded as intermediate, according to the Elston-Ellis modified Scarff-Bloom-Richardson grading system (SBR score 3 + 3 + 1 = 7 out of 9), histologic grade 2 on a scale of 1–3. In recent years fine-needle aspiration cytology (FNAC) is routinely being used for preoperative diagnosis of breast carcinoma.

Bloom-Richardson grading system—Synonyms: Modified Bloom-Richardson (BR), Scarff-Bloom-Richardson (SBR), SBR Grading, BR Grading, Elston-Ellis modification of Bloom-Richardson grading system. The Bloom-Richardson grading scheme is a semi- quantitative grading method based on three morphologic features of invasive (no-special-type) breast cancers. Over the years pathologists have modified SBR grading a bit. The Elston-Ellis, Black and Nottingham grade systems have the same basic principles as SBR to assign grades to breast cancer tumors.

In Europe, the Elston-Ellis modification of the SBR grading system (Nottingham grading system) is preferred and is becoming increasingly popular in the US (7). This modification provides somewhat more objective criteria for the three component elements of grading and specifically addresses mitosis counting in a more rigorous fashion (4).

RESULTS: Comparative analysis of SBR grade on aspirates from the peripheral portion and histopathology by the Pearson chi-square test (χ(2) =78.00) showed that it was statistically significant (P<0.001) with 93% concordance. Lower mitotic score on aspirates from the peripheral portion was observed in only 4 out of 45 (9%) cases.
